This pro-forma reply slip is a response addressed to the company from a shareholder in relation to the company’s demand letter to the shareholder to provide it with information for the register of members in accordance with section 113F Companies Act 2006.

This reply slip should be sent to each shareholder at the same time as the letter explaining the company’s power to require information from members. Failure to supply the information or the making of a false statement in relation to it, will be a criminal offence.
